Here's my two cents,
I have only been playing the game a few months and never played any of the previous versions. Played SP for a few months before joining this league (my first ever MP game) and also joined the APFL.
I really struggled in my first season with Portland but in the end an 8-8 season was decent as the roster was pretty horrible when I took over. I rexed for pretty much the whole season. The only adjustments I made was the distances in which my QB was throwing.
In the off-season I tried to improve the roster and my draft process is basically taking the guidelines from FOFC Forum. I also started to dip into gameplanning a bit more. So I was expecting maybe a 9-7 season but then lost the first four games and frustration starts to set in and you wonder what you need to do to catch a break. Now we're 5-6 but I can see us improving and I think a few more seasons of building and we will be a playoff team (hopefully for a few seasons).
I am now in my first season in the APFL after taking over the Cowboys and I have just reached the bowl game after going 14-4 on the season. This is a team that didn't reach the playoffs in the previous six seasons. I have an ageing QB in Big Ben, a good half back and decent receivers but overall it's a middle of the road roster. How have I got success? The offensive line is one of the best and this is the only thing I can think is the difference. My gameplan is mostly rexed with a few adjustments here and there but nothing special.
I think it takes time to build a top team and a bit of luck as well. Nemesis has lost 22 games in the last 3 seasons in the OSFL and only 3 of them have been by more than 10 points. Such a small margin between success and failure.
My only advice is stick at it, you are nearly there.
