Man, I'm of two minds about this. On the one hand there's something to be said for realism and on the other there are few things more frustrating than losing a key player in the preseason, and it'd be nice to be able to get through the preseason quickly...
I'm going to vote for a quick preseason with no injuries, but the alternatives wouldn't bother me.

On advice of the Commish, I deactivated most of my star players during the Preseason.. I missed one of my WR and he is out for 7 weeks..
Injuries seem realistic to me.. There are many real NFL stories of Stars going down in PS.. Or even before.. Look at MIn and their QB last year.. :(
My Question is in having your stars sit out all Pre Season will that make them a bit rusty? Like in Real Football? I hope that is the case and if so next Preseason I will play my stars more in the 3rd PS game which is what most NFL teams do as they consider that the dress rehearsal.
I dont vote for turning Injuries off. That's part of the game and GM process.. Even if it does suck..

I'm all for realism too but a 5 week preseason spread out so that we can make roster adjustments and not get players hurt/rusty or REX'd would take a week and a half or more.
I can can handle everyday SIMs for the regular season because it's only a move here or there. Preseason is a juggling act and owners get punished if we go fast.

The argument to completely turn injury's off for PS really makes no sense.. If you say turn injury's off for PS why would you not then say turn injury's off for the regular season? And Playoffs? And Super Bowl?
Injury's off for PS would be very unrealistic and feel very unrealistic..
Hearing that your starters would get rusty if you sit them all PS, I love that.. So the realistic balancing act during PS would come into play.. Believe me, as a Cowboy fan I hated it last year when Romo went down on the second play of PS1... Sure it became a blessing in disguise because of Dak but no one knew it at the time..
If I wanted Injury's off I would play Techno Bowl or NFL Blitz or some such..

Hoping the vote is 5 sims. I personally like the realism of injuries but lowering to 50 is cool. However it could be so low that only 1 player gets injured and it could be mine and yes I would be heartbroken. However here is the deal using my rookie QB for example. I would of been so sick seeing him go down to injury or even worse career ending injury in preseason. However I would be just as sick had it been week 1 or week 17. Couple seasons ago my 8th yr SLB Dante Park went down with a injury and his skills bottomed out. This happened right after I gave him a fat raise. Made me sick however it is life and it happens.. All that said I just enjoy game whatever the rule may be and I hope my team stays overall healthy. I am little worried about 2 yr FS Blackwell who last year went out with concussion.
